The wrong building
Our Revit model of 301 West Osborn had been built from the wrong building's drawings, so I called everything made before that date suspect.
A Revit model of 301 West Osborn was running, and for some time nobody saw that the machine had been pulling from a 1971 structural set for the building across the street. That was not the one we owned. I spotted it myself, comparing the model against the source documents by eye.
Seeing it, I did not patch the single error. I declared every model artifact made before that date suspect, and had the whole model purged. We rebuilt it from the original CAD files the architect had, and fixed the unit count in the process. Doing this to a project that already had momentum costs a lot. I did it anyway.
Find one bad foundation under a model and you should not hunt for the visible cracks and patch them. Treat everything downstream as contaminated until shown otherwise, and rebuild from the last point you can truly trust. Anything less is hoping the mistake was smaller than it was.
